What is CHA₂DS₂-VASc?

A score to predict stroke risk in atrial fibrillation. C=CHF, H=Hypertension, A₂=Age≥75 (2pts), D=Diabetes, S₂=Stroke/TIA (2pts), V=Vascular disease, A=Age 65-74, Sc=Sex category (female).

When should I anticoagulate?

Score 0 (male) or 1 (female): No anticoagulation. Score 1 (male): Consider anticoagulation. Score ≥2: Oral anticoagulation recommended.

Current guidelines (2019+) no longer recommend aspirin alone for AFib stroke prevention. If anticoagulation is indicated, use DOACs or warfarin.

Why do females get 1 point?

Female sex is a risk modifier, not an independent risk factor. A female with no other risk factors (score 1) doesn't need anticoagulation, but females with other risks have higher stroke rates.

Score 0: 0%. Score 1: 1.3%. Score 2: 2.2%. Score 3: 3.2%. Score 4: 4.0%. Score 5: 6.7%. Score 6+: 9.8%+.
