What complications does it predict?

MI, pulmonary edema, VF, cardiac arrest, complete heart block. Does NOT predict bleeding, DVT, or non-cardiac complications.

How should I use the results?

0 points: ~0.4% risk. 1 point: ~1%. 2 points: ~2.4%. ≥3 points: ~5.4%. Higher risk patients may benefit from preop echo, cardiology consult, or procedure deferral.

Does this apply to emergent surgery?

RCRI was derived for elective major non-cardiac surgery. Emergent surgery has inherently higher risk regardless of score.

Should all patients get preop stress testing?

No. Current guidelines recommend stress testing only if it will change management, typically for high-risk patients with poor functional capacity and planned surgery can be delayed.
